This article about the barriers faced while setting a follow up clinic for NICU "graduates" aptly underlines the complexity prevalent in health care system administration which creates complexity in execution of simple goal: complete well being of the patient. Although all the stake holders agree on the concept of patient centered care, but very few of them actually take steps to create or support a system which will fulfill this goal. The major hold off being reimbursement criteria to provide such services. More over this article also underlines the impact of administrative responsibilities being chopped up at different levels, preventing the health care system to move as single unit.
    An efficient way to bring change in the structure of health care delivery would be to introduce reimbursement reforms which would promote such practices at ground level and lure the higher authorities.
